What's the difference between a permit-required and non-permit confined space?

Permit-required = has hazardous atmosphere potential, engulfment potential, entrapment configuration, or other recognized hazards. Non-permit = confined space that meets the definition (limited entry/exit, not designed for continuous occupancy) but has NO hazards. Non-permit spaces don't need permits, but you still need to evaluate them and document the evaluation.

Do I need atmospheric testing before every entry?

Yes. 1910.146(d)(5) requires atmospheric testing prior to entry, and periodic re-testing during the entry when conditions may change. Order of testing: oxygen first, then flammables, then toxics. Testing must be done from outside the space when possible.

What are the acceptable atmospheric limits?

Oxygen: 19.5% to 23.5%. Flammable gas/vapor: below 10% LEL. Toxic contaminants: below permissible exposure limits (varies by substance). Any reading outside these = space is not safe for entry until conditions change.

Do I need a permit for a manhole in the street?

Can the attendant enter the space to help someone?

Only if the attendant is trained and equipped for entry rescue AND another qualified attendant takes their position outside. Non-entry rescue is preferred — retrieval line, tripod hoist. Untrained attendant entering to rescue is how confined space deaths become multi-fatality events.

What's the biggest confined space citation risk?
