Why Confined Space Kills More Workers Than It Should
Confined space fatalities have a horrible pattern: 60% of the deaths are would-be rescuers who entered without protection to help the first victim. Written permits and rescue plans exist specifically to prevent this cascade.
OSHA takes confined space seriously. When something goes wrong, the citation is often willful — because the standard has been on the books since 1993 and everyone knows the rules.
The 3 Elements That Make A Space "Permit-Required"
Under 1910.146(b), a confined space needs all 3 general traits:
- Large enough to enter and perform work
- Limited or restricted means of entry/exit
- Not designed for continuous occupancy
Then it becomes PERMIT-REQUIRED if it also has:
- Hazardous atmosphere (flammable, toxic, oxygen-deficient below 19.5% or enriched above 23.5%)
- Material that could engulf an entrant
- Configuration that could trap or asphyxiate (converging walls, sloped floors)
- Any other recognized serious safety or health hazard
What Must Be On A Confined Space Entry Permit
Under 1910.146(f), the permit must document:
Space Identification
- Specific location and description
- Purpose of entry (inspection, cleaning, repair, etc.)
- Date and duration of entry
Authorized Personnel
- Entry supervisor (signature)
- Authorized entrants (names)
- Attendants (names)
- Rescue service contact
Hazards And Controls
- Identified hazards
- Isolation procedures completed (LOTO, blanking, blinding)
- Ventilation type and equipment
- Atmospheric testing results (oxygen, LEL, toxic gases)
- PPE and rescue equipment required
Communication
- Method (radio, direct visual, tap signal, etc.)
- Rescue procedure summary
- Contact numbers
Atmospheric Testing
Must be done in this order:
- Oxygen (19.5-23.5% acceptable)
- Combustible gases (<10% LEL)
- Toxic gases (below PEL)
Retest at intervals appropriate to the operation. If conditions change, retest and re-permit.

Common Confined Space Mistakes That Kill
- No pre-entry atmospheric test — Or the test was done from outside, not inside the space.
- Untrained attendants — Attendant left post to "help" and became second victim.
- No rescue plan — "Call 911" is not a rescue plan. Fire department average response is 6-14 minutes. That's brain death in an oxygen-deficient atmosphere.
- Bypassed lockout/tagout — Line reactivated by someone unaware entry was happening.
- Old permit reused — Conditions changed since morning testing; new hazards not caught.
The Rescue Plan Requirement
1910.146(k) requires a written rescue procedure. Options:
1. In-House Rescue Team
Employer trains and equips its own rescue team. Requires annual practice in the actual space, medical monitoring, and full PPE including SCBA. Expensive but fastest response.
2. Contracted Rescue Service
Third-party emergency response contract. They must be evaluated for capability and response time. Local fire department may qualify ONLY if they have proper training and equipment specific to your spaces.
3. Non-Entry Rescue
Retrieval system (harness + retrieval line to mechanical device) that can extract an entrant without another person entering. Best option when feasible. Cannot be used if entrant is entangled or unconscious in an obstructed space.
